Crop your image to fit 100x100 and duplicated this layer setting it to overlay
Next take this gradient and paste it above all your layers and set this as exclusion.
Then you will end up with this
Create a new layer and add some tiny text
Under this layer using the rectangle tool, draw a white shape
Finally add some text and you're done ^^
